Zusammenfassung
Periprozedurale Komplikationen im Rahmen der diagnostischen Koronaragiographie oder auch im Kontext perkutan interventioneller Maßnahmen sind relativ selten, jedoch für den Patienten zum Teil lebensbedrohlich. Die geringe Inzidenz potenziell fataler Komplikationen (z. B. Koronarperforation) führt auch dazu, dass nicht jeder Operateur mit dem gesamten Spektrum therapeutischer Strategien (z. B. gecoverter Stent, Coiling) vertraut oder zumindest nicht routiniert darin geübt ist. Diese Arbeit vermittelt einen Überblick über mögliche periprozedurale Komplikationen bei koronarer Diagnostik und Therapie. Weiterhin werden, ausgehend von erweiterten ereignisgesteuerten Prozessketten (eEPK), praxisnahe Handlungsanweisungen in Form von „standard operating procedures“ (SOPs) und Checklisten zur Verfügung gestellt. Die Arbeit soll somit jedem invasiv kardiologisch tätigen Kardiologen ein belastbares Fundament für den rationalen Umgang mit Komplikationen im Katheterlabor bieten.
Abstract
Periprocedural complications during coronary angiography or percutaneous coronary interventions are relatively rare but potentially life-threatening events. The low incidence of fatal complications, e.g. coronary perforation, implies a relatively low level of routine experience regarding the management of these situations. The aim of this article is to provide a reference for periprocedural complications during coronary diagnostic and interventional procedures using standard operating procedures (SOPs) and checklists. The article thereby, provides the basis for a rational diagnostic and therapeutic approach for the most common complications in the catheter laboratory.
