Zusammenfassung

Diabetes mellitus ist eine klassische Alterserkrankung mit einer Prävalenz von ca. 20% in der Altersgruppe der 75- bis 80-Jährigen. Diabetes ist gerade bei alten Menschen mit typischen geriatrischen Symptomen wie eingeschränktem Sehvermögen, Inkontinenz, Depression und geistigem Abbau verbunden. Hinzu kommen oft auch noch motorische Funktionseinschränkungen und eine damit einhergehende erhöhte Sturzgefahr, sodass bei dieser sehr heterogenen Gruppe von Menschen besonders fokussiert individuelle Therapieziele vereinbart werden müssen. Höchste Therapieziele bei älteren Menschen mit Diabetes mellitus sind der Erhalt der Lebensqualität und die Vermeidung metabolischer Entgleisungen wie Hypo- oder Hyperglykämien und den sich daraus entwickelnden Konsequenzen. Daher wird z. B. in den Leitlinien der Deutschen Diabetes-Gesellschaft bei geriatrischen Patienten mit Diabetes ein HbA1c-Zielbereich zwischen 7 und 8% empfohlen. Die Besonderheiten der Diabetestherapie älterer Menschen mit Typ-2-Diabetes werden im vorliegenden Beitrag genauer betrachtet.

Abstract

Diabetes mellitus is a classical disease of the elderly with a prevalence of approximately 20% in the 75–80 years old age group. Diabetes is particularly in older people associated with typical geriatric symptoms, such as limitations in visual ability, incontinence, depression and mental deterioration. In addition motor functional limitations and an accompanying increase risk of falling are also often present so that a particularly focussed individualized therapy must be agreed for this very heterogeneous group of people. The primary aims of therapy in elderly people with diabetes mellitus are to maintain the quality of life and avoidance of metabolic derailment, such as hypoglycemia and hyperglycemia, and the resulting consequences. Therefore, in the guidelines of the German Diabetes Society, for example an Hb A1c target between 7 and 8% is recommended for geriatric patients with diabetes. The special features of diabetes therapy of elderly people with type 2 diabetes mellitus will be considered in detail in this article.
