Zusammenfassung

Die Qualitätssicherung bei ambulant erworbener Pneumonie („community acquired pneumonia“, CAP) wird seit mehr als 10 Jahren durchgeführt und erfasst alle in Deutschland stationär behandelten CAP-Patienten. Im Jahr 2015 waren dies 290.740 Patienten. Die Krankenhaussterblichkeit war mit 13,2 % um ein Vielfaches höher als die Sterblichkeit an Myokardinfarkt oder Schlaganfall. Mit Ausnahme der Sterblichkeit haben sich die Ergebnisse aller in diesem Verfahren genutzten Indikatoren über die Jahre gebessert. Neben der Sterblichkeit zeigen sich nur für drei Indikatoren Auffälligkeiten: die Frühmobilisierung sowie die Erfassung der Atemfrequenz und der klinischen Stabilitätskriterien. Erklären lässt sich die hohe Sterblichkeit dadurch nicht. Neue Verfahren, wie z. B. das Peer Review, müssen in Zukunft die Qualitätssicherung ergänzen, um das Management der Pneumonie im Krankenhaus zu verbessern.

Abstract

Quality assurance for community-acquired pneumonia (CAP) has been carried out for more than 10 years and covers all patients who are admitted to hospital for CAP in Germany. In 2015 this included 290,740 patients. The hospital mortality rate was 13.2% and is much higher than mortality from myocardial infarction or stroke. With the exception of mortality, the results of all indicators used in the quality management have improved over the years. In addition to mortality only three of the other indicators were conspicuous, i.e. early mobilization, measurement of the respiratory rate and clinical stability criteria; however, the high mortality rate is not explained by this. New procedures, such as the peer review process must complement quality assurance in the future in order to improve the management of pneumonia in hospitals.
