Tranexamsäure bei traumatischen und postpartalen Blutungen

“Alike but yet very different!”

Zusammenfassung

Im Frühjahr diesen Jahres wurden die Ergebnisse der WOMAN-Studie (World Maternal Antifibrinolytic Trial) veröffentlicht. Ähnlich dem Prinzip der CRASH-2-Trauma-Studie (Clinical-Randomisation-of-an-Antifibrinolytic-in-Significant-Haemorrhage-Trauma-Studie) erhielten zwischen 3/2010 und 4/2016 insgesamt 20.060 Frauen im Alter über 16 Jahre mit der klinischen Diagnose einer postpartalen Blutung nach vaginaler oder Kaiserschnittentbindung prospektiv-randomisiert entweder 1 g Tranexamsäure (TXA) intravenös oder Placebo in Ergänzung zur lokalen Standardtherapie. Im Ergebnis zeigte sich, wie in CRASH-2, ein ähnlich marginaler Überlebensvorteil zu Gunsten von TXA mit der gleichen Beschränkung des Therapieeffekts auf die frühe Gabe innerhalb von 3 h nach Entbindung: In der TXA-Gruppe wurden weniger Todesfälle infolge von Blutungen beobachtet als in der Placebogruppe. Wurde TXA innerhalb der ersten 3 h nach Entbindung verabreicht, war der Überlebensvorteil geringfügig höher. Obgleich der genaue Wirkmechanismus von TXA beim blutenden Traumapatienten noch immer nicht eindeutig geklärt ist, ist die Substanz eine sinnvolle Behandlungsoption, sofern die Indikation stimmt. Die breite Verfügbarkeit von TXA bei insgesamt günstiger Ökonomie und vermeintlich sicherem Anwendungsprofil sollte allerdings nicht über die gravieren Wissenslücken im Umgang mit der Substanz hinwegtäuschen und den liberalen und unkritischen Einsatz rechtfertigen.
