Indikationen und Grenzen des Telenotarztsystems

Zusammenfassung

Hintergrund

In der prähospitalen Notfallmedizin – wie auch in anderen medizinischen Bereichen – hat sich die telemedizinische Vernetzung zwischen medizinischem Personal und fachspezifischen Spezialisten als vorteilhaft für die Versorgungsqualität erwiesen. In der Notfallmedizin wurden jedoch bisher ausschließlich für das jeweils im Fokus stehende Krankheitsbild geeignete Telemedizinteilkomponenten genutzt. Daher wurde in Aachen weltweit erstmalig ein holistisches Telemedizinkonzept für das gesamte notfallmedizinische Spektrum als ergänzendes Strukturelement des existierenden boden- und luftgebundenen Rettungssystems entwickelt.

Material und Methode

Nach Klärung juristischer Aspekte, Erprobung und Weiterentwicklung wird das System nach Implementierung im Rettungsdienstbedarfsplan in Aachen krankenkassenfinanziert additiv seit dem 01.04.2014 in der prähospitalen Routinenotfallversorgung eingesetzt. Dieses aus Hard- und Softwarekomponenten bestehende System (Telenotarzt-Arbeitsplatz, Server-Infrastruktur, mobile und im Rettungswagen fest verbaute Übertragungseinheit) gewährleistet die Verfügbarkeit medizinischer Daten und den sicheren Datentransfer (Sprache, Echtzeit-Vitalparameter-Kurven und Werte, Fotos, Videostream, 12-Kanal-EKG etc.) in Echtzeit.

Ergebnisse

Aus den bisher insgesamt ca. 5000 Behandlungsfällen in zunächst Forschung und dann Regelversorgung lassen sich klare Indikationen und Grenzen dieses Systems ableiten.

Schlussfolgerungen

Die bisherigen telemedizinischen Einsätze konnten sicher durchgeführt werden, belegen im Vergleich zum konventionellen Notarztsystem eine bessere Anamnese-, und Dokumentationsqualität, verkürzen die ärztliche Bindungszeit um 50 %, reduzieren die Gesamtzahl der ärztlich begleiteten Rettungsdiensteinsätze und zeigen eine mindestens gleichwertige medizinische Versorgungsqualität.
