Die Bedeutung der endothelialen Glykokalyx für die Infusionspraxis in der Notfallmedizin

Zusammenfassung

Die endotheliale Glykokalyx ist ein entscheidender funktioneller Bestandteil der vaskulären Barriere. Ischämie, Entzündung und Hypervolämie sind u. a. in der Lage, ihre Integrität zu gefährden. In der elektiven perioperativen Situation setzt man diese relativ neuen Erkenntnisse durch eine verlustorientierte Infusionspraxis zunehmend um. Der traumatisierte Notfallpatient hingegen eignet sich hierfür in der Regel weniger: Seine Flüssigkeitskompartimente sind oft nicht im Gleichgewicht und das kardiozirkulatorische Monitoring ist begrenzt. Zudem ist das große Trauma ein massiver inflammatorischer Reiz, der die endotheliale Glykokalyx bereits zu einem Zeitpunkt zerstören dürfte, der sich therapeutischen Bemühungen entzieht. Ziel notärztlicher Bemühungen beim kreislaufinstabilen Patienten ist die unverzügliche Wiederherstellung eines adäquaten Herzzeitvolumens unter Aufbietung sämtlicher präklinisch verfügbarer Möglichkeiten. Glykokalyxprotektion muss hier zurückstehen und käme wahrscheinlich in der Regel ohnehin zu spät.
