Evidenzbasierte Notfallmedizin – Status quo

Zusammenfassung

Die Notfallmedizin kann als interdisziplinäres Fach auf die wissenschaftlichen Erkenntnisse verschiedener medizinischer Disziplinen zurückgreifen. Spezifische notfallmedizinischer Studien sind jedoch unverzichtbar, um notfallmedizinisches Handeln auf der Basis evidenzbasierten Wissens zu ermöglichen. Die Planung und Durchführung notfallmedizinischer Studien muss spezielle ethische und finanzielle Aspekte berücksichtigen. So sind Notfallpatienten nicht uneingeschränkt einwilligungsfähig, und die Notwendigkeit großer randomisierter Studien bringt einen erheblichen Finanzierungsaufwand mit sich, der ohne ausreichende finanzielle Unterstützung durch Industrie und andere Institutionen nicht abzudecken ist. Auf der Basis notfallmedizinischer Studien lassen sich „standard operating procedures“ (SOP) erstellen, die evidenzbasierte Therapiealgorithmen klinisch umsetzbar machen. Anhand zweier großer notfallmedizinischer Themenkomplexe (kardiopulmonale Reanimation und Polytraumaversorgung) wird verdeutlicht, dass aktuelle Handlungsempfehlungen aus evidenzbasiertem Wissen gut geplanter und durchgeführter klinischer Studien hervor gehen. Dabei sind Studien, die neue Therapieansätze beleuchten, von besonderem Interesse. Daneben sind jedoch auch Studien von besonderer Bedeutung, welche die Effizienz bestehender, teils „historischer“ oder aus theoretischen Überlegungen abgeleiteter Therapiekonzepte evaluieren.
