Therapeutische Hypothermie nach Herz-Kreislauf-Stillstand—quo vadis?

Zusammenfassung

Level-I-Evidenz unterstützt die Anwendung der therapeutischen Hypothermie nach prähospitalem Herz-Kreislauf-Stillstand und Kammerflimmern bzw. pulsloser Ventrikeltachykardie als initialem EKG-Rhythmus. Obwohl die Empfehlung zum Einsatz der therapeutischen Hypothermie schon vor 2 Jahren in internationalen Richtlinien ausgesprochen wurde, erfolgt die Umsetzung in die klinische Praxis, wie Untersuchungen zeigen, schleppend. Der vorliegende Beitrag erläutert die Auswirkungen dieser mangelnden Implementierung hinsichtlich des neurologischen Outcomes der Patienten. Verschiedene gebräuchliche Kühlverfahren sowie die häufigsten unerwünschten Wirkungen der therapeutischen Hypothermie werden dargestellt.
