Einsatz von Hilfeleistungslöschfahrzeugen als First-Responder-Einheiten

Zusammenfassung

Material und Methoden.

Der prospektiv untersuchte Rettungsdienstbereich versorgt etwa 1,5 Millionen Menschen mit einem gestaffelten System aus RTW und NAW/NEF. Jährlich werden etwa 1.000 Reanimationen durch den Rettungsdienst durchgeführt. Seit 1997 erfolgte nach Ausbildung von 1500 Feuerwehrleuten im Gebrauch von automatisierten externen Defibrillatoren (AED) der Einsatz von Hilfeleistungslöschfahrzeugen ( HLF) als First-Responder-Einheiten (FRE) zunächst in Form von Machbarkeitsstudien. Im Jahr 2000 wurden HLF regelhaft als zusätzliches 3. Glied des Rettungsdienstes neben RTW und NAW/NEF beim Meldebild Reanimation eingesetzt. Im Jahr 2001 wurde die Indikation auf Bewusstlosigkeit, Kreislaufstillstand, Atemstillstand, Polytrauma und „andere akute Lebensbedrohung“ erweitert.

Ergebnisse.

Die FRE wurden im Jahr 2000 zu 427 Einsätzen disponiert, bei 43 Patienten lag ein Herzkreislaufstillstand vor (4,5% der 966 Reanimationen in 2000). Bei erweiterten Einsatzindikationen im Jahr 2001 führten die HLF 3763 Einsätze durch, in 3035 Fällen bei „Bewusstlosigkeit“ (80,6%), in 212 Fällen bei „Kreislaufstillstand“ (5,2%). Bei Erweiterung der Alarmierung, insbesondere wegen „Bewusstlosigkeit“, wurden 312 reanimationspflichtige Patienten angetroffen (32,8% der 952 Reanimationen in 2001). Ein AED wurde bei 148 Patienten eingesetzt (8,2%), die Defibrillation erfolgte in 55 Fällen (3,0%). Bei 1792 Einsätzen trafen die HLF vor RTW oder NAW/NEF beim Patienten ein (47,6%), die Eingreifzeiten wurden um 3 min verkürzt.

Schlussfolgerung.

Hilfeleistungslöschfahrzeuge können auch bei hoher Verkehrsdichte effektiv als First-Responder-Einheiten eingesetzt werden. Dabei erfasst die Disposition auf das Meldebild Bewusstlosigkeit jede 3. Reanimation. Bei jedem 2. Einsatz trafen die First Responder der Feuerwehr vor dem Rettungsdienst ein, damit können bei geringem finanziellem Einsatz die Eingreifzeiten beim präklinischen Herzkreislaufstillstand wesentlich verkürzt werden.
