Spontanruptur eines Interkostalarterienaneurysmas mit Hämatothorax bei Neurofibromatose Typ 1

Zusammenfassung

Ein 46-jährigen Patient mit bekannter Neurofibromatose Typ 1 (NF Typ 1) stellte sich in einem auswärtigen Krankenhaus mit thorakalen Schmerzen und Dyspnoe vor. Die konventionelle Röntgenuntersuchung des Thorax zeigte einen ausgedehnten Pleuraerguss mit kompletter Verschattung des rechten Hemithorax. Der Patient wurde im kreislaufstabilen Zustand mit einem Hämoglobinwert von 4,0 mmol/l zu uns verlegt. Die Computertomographie demonstrierte eine gedeckt perforierte Ruptur eines Aneurysmas der rechten 9. Interkostalarterie ohne aktive Blutung und einen ausgedehnten rechtseitigen Hämathothorax. Das Aneurysma wurde endovaskulär mit Coils embolisiert. Nachfolgend führten wir eine anterolaterale Thorakotomie rechts mit Ausräumung eines ausgedehnten extrapleuralen Hämatoms durch. Anhand dieses Fallberichts diskutieren wir die Diagnostik und Therapie der mit der NF Typ 1 assoziierten Gefäßaneurysmata.

Abstract

A 46-year-old male with neurofibromatosis type 1 presented to hospital with chest pains and dyspnea. A chest X-ray revealed a large effusion and complete shadowing of the right hemithorax. The patient was transferred to our clinic in a stable hemodynamic condition with a hemoglobin value of 4.0 mmol/l. A computed tomography (CT) scan revealed a spontaneous rupture of an aneurysm of the right 9^th intercostal artery without signs of active bleeding and an extensive right-sided hematothorax. The aneurysm was treated by endovascular embolization. An anterolateral thoracotomy with removal of a massive hematoma was subsequently performed. In this report we discuss the diagnostics and therapy of vascular aneurysms associated with neurofibromatosis type 1.
