Zusammenfassung

Einleitung

Mit der Zunahme der kardiovaskulären Erkrankungen steigt die Anzahl arterieller Punktionen. Es werden vermehrt arterielle Verschlusssysteme eingesetzt.

Patienten und Methoden

Von 01.01.2004–31.12.2004 traten 13 operationspflichtige Komplikationen nach Anwendung arterieller Verschlusssysteme (12-mal Angioseal(r), 1-mal Perclose(r)) auf.

Ergebnisse

Die 13 Patienten (m. : w./7:6) waren durchschnittlich 64,6±10,2 Jahre alt. 11-mal war die Komplikation ein Verschluss, 2-mal eine arterielle Blutung. Rekonstruiert wurde 11-mal durch Thrombendarteriektomie, 2-mal mit Hilfe einer Gefäßnaht. Die Operationszeit betrug 87,3±30 min, die Intensivzeit 25 h und der stationäre Aufenthalt 10,8 Tage. Alle Extremitäten konnten erhalten werden. Der Verschluss war 8-mal (72%) kombiniert und 3-mal isoliert. Der zentrale Einstrom über die Aorta und die Beckenstrombahn war nicht eingeschränkt.

Schlussfolgerung

Die alleinige Embolektomie als chirurgische Maßnahme ist nicht ausreichend. Die Wundheilung ist wegen der oft adipösen Patienten mit starkem Reperfusionssyndrom und vorpunktierter Leiste problematisch und verzögert den stationären Aufenthalt.

Abstract

Introduction

The increasing number of arterial punctures for angiography has led to the widespread use of vascular closure devices.

Patients and methods

Thirteen complications after the use of vascular closure devices on 5,000 patients were recorded from the beginning of January 2004 until the end of December 2004 (12× Angioseal(r), 1× Perclose(r)).

Results

These patients, seven males and six females, had a mean age of 64.6±10.2 years. The complication involved an arterial occlusion in 11 cases and arterial bleeding in the other two. Endarterectomy had to be used for arterial occlusion, vascular suture for the bleeding. The operating time was 87.3±30 min, the ICU period 25 h, and the hospital stay 10.8 days. All extremities could be saved. In eight cases, the occlusion involved more than one femoral vessel.

Conclusion

Embolectomy is not able to save extremities in cases of occlusion after the use of vascular closure devices. The complexity of the damage to the arteries makes endarterectomy necessary.
