Zusammenfassung

In den letzten Jahrzehnten hat sich die Versorgung psychisch Kranker in einem Ausmaß verändert wie in keinem anderen Fachgebiet. Die Zahl psychiatrischer Betten wurde im Vergleich zu den 70-er Jahren des letzten Jahrhunderts in Österreich auf weniger als ein Drittel reduziert. Parallel dazu stieg aber die Zahl der Betten zur Behandlung psychisch Kranker in anderen Bereichen, beispielsweise in der forensischen Psychiatrie, deutlich an. Die Verlagerung von Betten aus den psychiatrischen Krankenhäusern in Allgemeinkrankenhäuser fand in vielen europäischen Ländern statt. Angesichts der Tatsache, dass die meisten psychisch Kranken auch unter körperlichen Krankheiten leiden und die somatische Komorbidität den wichtigsten Grund für die verkürzte Lebenserwartung psychisch Kranker darstellt, ist dies eine äußerst positive Entwicklung. Das „Assertive Community Treatment“, eine intensive nachgehende ambulante Versorgung unter Einbeziehung von Psychiatern, ist gerade für schwer und chronisch psychisch Kranke von Vorteil.

Abstract

During the last decades the health care for the mentally ill has changed dramatically. Since the 1970s the number hospital beds for psychiatric patients was reduced in Austria to less than one third. Nevertheless, during the same period the number of beds for psychiatric patients increased in other areas of the healthcare system, such as in forensic psychiatry. In many European countries, psychiatric hospital beds were displaced from old mental hospitals to psychiatric departments in general hospitals. In view of the fact that the majority of mentally ill persons also suffer from physical diseases and that the somatic comorbidity is the main reason for the reduced life expectancy of the psychiatrically ill, this is an extremely positive development. Assertive community treatment is a comprehensive outpatient treatment including medication provided by psychiatrists and has numerous advantages especially for persons with severe and chronic mental disorders.
