Zusammenfassung

Derzeit gibt es in Österreich kein in allen Bundesländern einheitlich etabliertes Betreuungsmodell eines „Heimarztes“ für Pflegeheime. Im Bundesland Wien werden seit mehr als 100 Jahren chronisch Kranke und hochgradig pflegebedürftige Menschen in den städtischen Pflegeeinrichtungen und in einigen Institutionen geistlicher Träger rund um die Uhr von angestellten geriatrisch versierten ÄrztInnen betreut. Die Rechtsform dieser Häuser entspricht prinzipiell der einer „Pflegeanstalt für Chronisch Kranke“ nach dem Österreichischen Krankenanstaltengesetz. Aber auch andere Träger in Wien und Niederösterreich beschäftigen angestellte ÄrztInnen an Bettenstationen von Pflege- und Wohnheimen. Der Bedarf nach einer kontinuierlichen, fachlich spezialisierten ärztlichen Betreuung ergibt sich vor allem aus der Notwendigkeit der Therapie psychiatrischer Erkrankungen im hohen Alter mit Psychopharmaka, der komplexen Multimorbidität und Polypharmazie, sowie aus dem Bedarf nach rehabilitativen Maßnahmen bzw. palliativer Begleitung am Lebensende. Mit dieser Aufgabenstellung sind geriatrisch nicht adäquat ausgebildete bzw. nicht in den Pflegeheimen integrierte ÄrztInnen häufig überfordert. Die Konsequenzen davon sind häufige Spitalseinweisungen bzw. suboptimale ärztliche Betreuung. Als notwendige Maßnahmen zur Qualitätssicherung in geriatrischen Pflegeeinrichtungen ist daher bundesweit die Implementierung von qualifizierten HeimärztInnen nach niederländischem Vorbild zu fordern.

Abstract

Definition of the problem

Nowadays there is no universal model of a “nursing home doctor” implemented in Austria. For more than 100 years, the city of Vienna has employed doctors to treat continuously chronic ill and care-dependent patients in its Viennese long-term care institutions. In other federal states of Austria, such as Lower Austria, in some nursing homes employed doctors are also working for the residents.

Arguments and conclusion

There is a need for continuous specialized care that results from gerontopsychiatric disorders treated with psychotropic medication and from multi-morbidity and consequent multi-medication. Moreover, nursing home doctors can deliver necessary rehabilitative and palliative therapy if necessary. Non-specialized doctors sometimes can not cope with these challenges sufficiently. Thus, frequent transfer to a hospital or medical under-treatment is the consequence. Therefore, the implementation of a nation-wide “nursing home doctor” model analogous to that in the Netherlands would help to set up new quality standards for quality of medical care in nursing homes.
