Zusammenfassung
Meine 1999 veröffentliche Theorie „Psychotherapie als Profession“ wird Grundlage zu einer Reflexion auf den Stand von heute: Die psychotherapeutische Forschung hat die „talking cure“ in ihren Effektstärken und „Outcome“-Maßen erheblich rehabilitiert; weder die technische („Interventionen“ bei „Störungen“) noch die medizinische Metapher (Stiles und Shapiro 1989), haben halten können, was sie versprachen; „Randomized-controlled-trial“-(RCT)-Methodologie hat strenge Logik, weniger jedoch Praxisrelevanz für sich. Diese Forschungsumwelt kontrastiert mit einer politischen Umwelt, wie sie durch eine „Direktausbildung“, vom Psychotherapeutentag im Herbst 2015 beschlossen, geschaffen würde. Sie würde medizinische Orientierungen in die Ausbildung bringen, wie sie derzeit in der Forschung infrage gestellt werden.
Andere scharfe Konfliktlagen sind: Zwar gibt es Frontlinien zwischen verschiedenen therapeutischen Schulen, vor allem Verhaltenstherapie (VT) bzw. Cognitive Behavior Therapy (CBT) und psychodynamischen Richtungen. Doch verläuft die wichtigere Frontlinie der „psychotherapy wars“ (Woolfolk 2015, S. 34 spricht sogar von „civil wars“) zwischen einer humanwissenschaftlichen und einer technisch-pharmakologischen Auffassung der Psychotherapie. Die Erfolge der Letzteren sind fragiler als ihre Propaganda. Was bleibt und wirkt, ist eine humanwissenschaftliche, durchaus auch humanistische Rehabilitierung therapeutischer Orientierungen, die in professioneller Konversation vermittelt werden. Die Profession könnte in diesem günstigen historischen Augenblick die Freiheit zurückgewinnen, humanwissenschaftliche Orientierung und therapeutische Gesprächskunst selbst ins Gespräch zu bringen. Die alte Frontstellung zwischen „Hermeneutikern“ und „Empirikern“ kann durch eine dritte Position überwunden werden. Es gibt eine neue, eine „andere Empirie“: die des Gesprächs.
Abstract
My theory of “Psychotherapy as profession” published in 1999 forms the foundations of reflections on the current status: psychotherapy research has considerably rehabilitated the talking cure in its effect strengths and outcome measures. Neither the technical (interventions for disorders) nor medical metaphors (Stiles and Shapiro 1989) could uphold what they promised: randomized controlled trial (RCT) methodology involves strict logic but has less practical relevance. This research environment contrasts with a political environment as achieved by a direct training, as decided by the psychotherapist convention in autumn 2015. This would bring medical orientation into the training, as is currently being questioned in research.
Other sharp conflict situations are: front lines do exist between various schools of therapy, especially behavior therapy (BT) or cognitive behavior therapy (CBT) and psychodynamic schools of thought; however, the more important front line of “psychotherapy wars” (Woolfolk 2015, p.34, even talks of “civil wars”) runs between a human scientific and a technical pharmacological perception of psychotherapy. The successes of the latter are more fragile than its propaganda. What persists and functions is a human scientific, indeed even humanistic rehabilitation of therapeutic orientations, which are imparted in professional conversation. In this favorable historic moment the profession could reclaim the freedom of bringing human scientific orientation and the therapeutic art of conversation into discussion. The old front position between “hermeneutics” and “empiricists” can be overcome through a third position. There is a new and “other empiricism”: that of conversation.

Der Chef der Techniker Krankenkasse gesteht die Relevanz dieses Themas sogar für den Bereich der organischen Medizin zu; Abrechnungssysteme erzeugen Systemzwänge, die dann zum Eindruck von „mehr Krankheiten“ führten (FAZ vom 14.10.2016, S. 15).
So berichtete die Irish Times am 5. Oktober 2003. Ein spezielles Heft von PLoS medicine (April 2006) beschäftigt sich gar mit der „Erfindung von Krankheiten“ unter dem Einfluss der Pharma-Industrie (Lexchin 2006; Phillips 2006). Es geht darum, wie eine gewöhnliche gelegentliche Störung, etwa erektile Dysfunktion zu einer „Krankheit“ umdefiniert und damit dem medizinischen Regime unterworfen wird. Ähnliches bei „attention deficit hyperactivity disorder“ (ADHD) oder auch bei „selling bipolar disorder“ (Healy 2006). Dass solche provokanten Titel in einer der angesehensten Zeitschriften der medizinischen Welt publiziert werden, zeigt, wie brisant das Thema empfunden wird. Andere Autoren hatten darauf hingewiesen, wie in Psychotherapie und Psychiatrie Trauer beim Verlust eines Partners als behandlungsnotwendige „Krankheit“ definiert wird (Horvitz und Wakefield 2007) oder das, was früher einfach „Schüchternheit“ hieß, in behandlungsbedürftige „Soziophobie“ umdefiniert wurde (Lane 2007). So wurden mit der Zahl der Krankheiten die Zahl der Patienten vermehrt und der Verkauf von Medikamenten gefördert.
Horst Kächele hat deshalb begonnen, nachdrücklich „Fehlerkultur“ zu untersuchen – die Schaffung von Gesprächsatmosphären in Supervisionen vor allem, aber auch von Studien, die Fehler überhaupt einzugestehen erlauben (Kächele und Grundmann 2011; Kächele und Caspar 2012). Das wurde von anderen übernommen (Zwiebel 2014).
