Zusammenfassung

Die rechtzeitige operative Therapie von Trikuspidalklappen(TK)-Vitien, insbesondere im Rahmen von Kombinationseingriffen, kann die Lebensqualität und die Prognose der betroffenen Patienten verbessern, ohne dabei das Operationsrisiko signifikant zu erhöhen. Neben den vergleichsweise seltenen primären Vitien der TK, die entweder angeboren oder durch eine Endokarditis erworben sein können, stellen auch schwere sekundäre Vitien, bedingt durch eine Dilatation des TK-Anulus, eine Operationsindikation dar. Zur präoperativen Diagnostik ist die Echokardiographie am besten geeignet. Neben der Morphologie und der Funktion der TK sollten die Funktion des rechten Ventrikels sowie Begleitvitien untersucht werden. Zur chirurgischen Therapie stellt die TK-Rekonstruktion durch eine Anuloplastie mithilfe von Naht oder Anuloplastiering das Verfahren der Wahl dar. Selten ist der Trikuspidalklappenersatz (TKE) erforderlich; aufgrund des geringeren Thromboembolierisikos sollten dabei biologische Prothesen bevorzugt werden.

Abstract

Timely operative treatment for tricuspid valve diseases, especially as a combined procedure can improve the quality of life and prognosis of affected patients without a relevant increase in the operative risk. Apart from the comparatively rare situation of primary tricuspid valve failure, which can either be congenital or caused by endocarditis, the main indication for surgery is the much more frequently occurring secondary tricuspid valve failure, caused by dilatation of the tricuspid valve annulus. For preoperative evaluation echocardiography is considered the method of choice. Besides, the morphology and function of the tricuspid valve, right heart function and concomitant valve disorders have to be investigated. For tricuspid repair suture and ring anuloplasty are currently the techniques of choice. Tricuspid valve replacement is rarely required but if so a biological prosthesis should be preferred due to the lower thromboembolic risk profile.
