Zusammenfassung
Die Betreuung von Patienten mit systemischem Lupus erythematodes (SLE) erfordert eine regelmäßige umfassende Diagnostik, die das Erfassen neuer Krankheitszeichen, der Krankheitsaktivität, behandlungsbedürftiger Komorbiditäten und insbesondere die Suche nach begleitenden Infektionen beinhaltet. Die immunsuppressive Therapie richtet sich an den aktiven Organmanifestationen und der allgemeinen Krankheitsaktivität aus, die mit standardisierten Scores erfasst werden können. Für diese Einschätzung ist es wichtig, Aktivität von Schaden zu differenzieren, da letzterer vor allem einer symptomatischen Therapie bedarf. Beide zusammen ermöglichen die Prognoseabschätzung, für die ansonsten kaum geeignete Surrogatmarker zur Verfügung stehen. Die Standardformen der immunsuppressiven Therapie des SLE haben sich in den letzten Jahren nicht geändert, die Strategien sind jedoch neu. Heute besteht prinzipiell bei jedem Lupus-Patienten die Indikation zur Behandlung mit Antimalariamitteln. In der Behandlung der proliferativen Lupus-Nephritis wird der Einsatz von Cyclophosphamid auf die Induktionsphase der Therapie beschränkt.
Abstract
Standard care of patients with systemic lupus erythematosus (SLE) covers continuous extended diagnostics including the search for new disease manifestations, evaluation of disease activity and of comorbidities requiring treatment and, in particular, accompanying infections. Immunosuppressive medication is determined by active organ manifestations and global disease activity, which is measured by standardized scores. In this context, it is important to differentiate between activity and damage, since damage is mostly treated symptomatically and, by definition, cannot be changed by immunosuppression. Activity and damage are the keystones estimating prognosis, especially since validated surrogate markers are lacking. The standard forms of immunosuppressive therapy for SLE have not changed in recent years, but the strategies are new. Antimalarials are indicated in every lupus patient as long as there are no contraindications. In proliferative nephritis, cyclophosphamide should be limited to the induction phase.
