Ambulante Palliativversorgung aus Sicht der Pflege in Vorarlberg, Österreich

Zusammenfassung

Hintergrund

Der Bedarf an „palliative care“ in der häuslichen Pflege wird aufgrund der demografischen Entwicklung in den kommenden Jahren zunehmen und Hausärzte wie auch Pflegedienste der Grundversorgung vor eine Herausforderung stellen.

Fragestellung

Untersucht wird der Stand der ambulanten Palliativversorgung in Vorarlberg aus Sicht der ambulanten Pflegedienste (Krankenpflegevereine, KPV). Kernpunkte der Befragung umfassen die Symptombelastung von Palliativpatienten sowie die Kooperation und Kommunikation der KPV mit Hausärzten.

Methode

Mittels eines Online-Surveys wurden zwischen 08.02.2016 und 07.03.2016 alle 50 ambulanten Pflegedienste Vorarlbergs (KPV) in einer quantitativen, deskriptiven, explorativen Untersuchung zu einer Einschätzung der ambulanten Palliativversorgung eingeladen. Insgesamt 38 Pflegedienste (76 %) haben den Fragebogen beantwortet.

Ergebnisse

Der Anteil der Patienten, die nach Einschätzung der KPV Palliative care bedürfen, beträgt 5,7 %. Die Bedarfseinschätzung korreliert mit dem Ausbildungsgrad (p = 0,02). Die Pflegekräfte geben eine hohe Symptomlast der zu Betreuenden (76 %) an. Zur Kommunikation zwischen Arzt und ambulantem Pflegedienst werden am häufigsten Telefonkontakte (79–90 %) genutzt, wenig gemeinsame Visiten (13–21 %) und Fallbesprechungen (26–45 %). Die Rate der stationären Einweisungen könnte nach Meinung von 63 % der Pflegedienste bei besserer interprofessioneller Zusammenarbeit reduziert werden.

Schlussfolgerung

Die Symptomlast ambulant betreuter Menschen ähnelt der im Krankenhaus betreuter Patienten. Das Gesundheitspersonal in der allgemeinen Palliativversorgung sollte entsprechend ausgebildet sein. Die Qualität der Kommunikation via Telefonate sollte in weiterführenden Studien kritisch beleuchtet werden.
