Wirksamkeit präventiver Hausbesuche bei älteren Menschen

Systematische Bewertung der aktuellen Literatur

Hintergrund

Präventive Hausbesuche (PHB) erfassen gesundheitliche Ressourcen und Risiken in der häuslichen Umgebung, um präventive Empfehlungen abzuleiten. In einer früheren Übersichtsarbeit fand sich kein sicherer Beleg für ihre Wirksamkeit bei älteren Menschen. Präventive Hausbesuche werden in Deutschland jedoch zunehmend als Leistung der kommunalen Daseinsfürsorge eingesetzt.

Ziel der Arbeit

Dieser Beitrag bewertet die Wirksamkeit von PHB anhand evidenzbasierter Kriterien.

Material und Methode

Mithilfe einer Recherche in relevanten Literaturdatenbanken wurden nach 2004 veröffentlichte randomisierte kontrollierte Studien (RCT) und systematische Übersichtsarbeiten zu anlassunabhängigen PHB bei Älteren (60+ Jahre) identifiziert. Ausgewertete Endpunkte waren Mortalität, Krankenhaus‑, Pflegeheimaufnahme, funktioneller Status, Sturz und Lebensqualität.

Ergebnisse

Die eingeschlossenen 12 RCT (darunter erstmals eine deutsche Studie) wiesen hohe klinische Heterogenität, gute methodische Qualität und nur vereinzelt Verzerrungspotenzial auf. Lediglich in 6 der 12 RCT fanden sich positive Effekte für einzelne Endpunkte, selten über den Interventionszeitraum hinaus, teilweise begrenzt auf einzelne Subgruppen. Negative Effekte wurden hingegen kaum berichtet. Auch die 4 einbezogenen Übersichtsarbeiten zeigten keine sicheren Belege für eine Wirksamkeit von PHB auf einen der ausgewerteten Endpunkte.

Schlussfolgerung

In der aktuellen Literatur fanden sich keine eindeutigen Wirksamkeitsbelege für PHB. Es ließen sich auch keine Struktur- und Prozessmerkmale mit Wirksamkeit auf mehrere Endpunkte ableiten. Die derzeitige Anwendung von PHB erfolgt somit unverändert ohne eindeutigen Wirksamkeitsnachweis.
