Candida-Infektionen im hohen Alter

Zusammenfassung

Im hohen Alter nehmen Candida-Infektionen an Häufigkeit und Schweregrad zu. Es treten verschiedene Manifestationen auf der Haut und den Schleimhäuten sowie in inneren Organen auf. Dafür gibt es eine Reihe von biologischen Gründen, z. B. die Veränderungen in der Immunabwehr alter Menschen. Zwar ist Candida albicans am häufigsten zu finden, aber Infektionen mit Candida glabrata haben zugenommen. Candida glabrata bedingt eine erhöhte Mortalität und spricht schlechter auf Fluconazol an, sodass dieses Antimykotikum bei alten Menschen nicht mehr das Mittel der 1. Wahl für eine kalkulierte Therapie ist. Grundsätzlich müssen bei der Wahl der Therapeutika die möglichen Nebenwirkungen und Interaktionen der verschiedenen Antimykotika berücksichtigt werden. Echinocandine haben wegen ihrer guten Verträglichkeit und geringen Interaktionsneigung sowie ihrer breiten und guten Wirkung gegen Sprosspilze im Alter einen hohen Stellenwert.
