Das akute postanoxische Koma: Klinische, elektrophysiologische, biochemische und bildgebende Befunde

Summary

Prognostic assessment of acute global cerebral anoxia following cardiorespiratory arrest requires¶a high level of clinical and technical diagnostic skills. Statements regarding prognosis of this form of coma can be made only with reference to¶a large number of clinical, neurophysiological, biochemical, and cerebral imaging data. Previous research in this field has usually been restricted to only one or a few parameters such as clinical data, EEG findings, evoked potentials or biochemical markers. The present compilation of the various aspects regarding diagnosis and prognosis is intended to define the present state of the art in diagnosis and prognosis of anoxic coma, and also serve as a basis for a multicenter prospective study including all of the relevant data to improve diagnosis and arrive at guidelines for effective prognostication of anoxic coma.

Zusammenfassung

Die Einschätzung der akuten globalen Anoxie des Gehirns nach akutem kardiorespiratorischem Versagen stellt hohe Anforderungen an die klinische und apparative Diagnostik. Die prognostische Einschätzung dieser Komaform kann nur unter Beachtung einer Vielzahl von klinischen, neurophysiologischen, biochemischen und Daten bildgebender Verfahren erfolgen. Die bisherigen Untersuchungen zu diesem Thema sind meist nur unter Berücksichtigung einzelner Parameter vorgenommen worden. Die Zusammenstellung der verschiedenen Aspekte der Diagnostik und Prognostik soll einerseits den derzeitigen Wissenstand darstellen und andererseits als Basis für eine multizentrische Erhebung beim akuten anoxischen Koma dienen, bei der die prognostisch relevanten Daten prospektiv und im Verlauf erhoben werden sollen.
