Zusammenfassung

Notfall- und Intensivmediziner werden zunehmend mit Situationen konfrontiert, in denen Patienten betreut werden müssen, für die keine kurativen Therapieoptionen mehr bestehen. Der Grund dafür sind epidemiologische Entwicklungen und eine zunehmende Abgabe von medizinischen Entscheidungen am Lebensende an notfallmedizinische Dienste und Intensivstationen. Schwierige Entscheidungen unter Zeitdruck führen in der Notfallmedizin überdies häufig zu einer maximalen Ausschöpfung des Machbaren, ohne die Option einer palliativmedizinisch orientierten Vorgehensweise zu erwägen. Für die Intensivmedizin können sich palliativmedizinische Fragestellungen bei der Aufnahme ebenso wie bei der Thematik der Therapielimitierung ergeben. Palliation ist genau betrachtet eine Grundkonstante der Intensivmedizin. Palliative Handlungsansätze müssen daher als Erweiterung des notfallmedizinischen Spektrums und als eine essenzielle Zielsetzung in der Intensivmedizin anerkannt werden. Dies muss als Aufgabe für die tägliche Praxis und die Weiterentwicklung dieser Fachdisziplinen gesehen werden.

Abstract

Emergency physicians and intensivists are increasingly faced with situations requiring them to care for patients for whom no curative treatment options are available. This is due to the epidemiologic evolution as well as the trend to delegate end-of-life decisions to emergency services and intensive care units. Difficult decisions under time pressure in emergency medicine frequently induce a response to use the full potential of feasible actions but without consideration of palliative treatment options. In intensive care the issue of palliative care might arise during decisions about admission of a patient as well as limiting treatment in the intensive care unit. In fact palliative care can be seen as a constant part of intensive care medicine. Palliative treatment concepts should be recognized as an expansion of the spectrum of emergency medicine and an essential goal in intensive care medicine. This is a task for daily practice and the further development of both medical specialities.
