Summary
The case report describes the successful thrombolysis with reteplase (Rapilysin®) of a 66-year-old woman with massive pulmonary embolism and cardiopulmonary resuscitation prior to admission to the clinic. The patient with no cardiac history was found unconscious with apnoe and cardiac arrest. Pulse was found only under cardiopulmonary rescusitation and directly after injection of suprarenin. Otherwise with regular sinus tachycardia electromechanical splitting could be observed. Since no stabilization could be achieved by this procedure the emergency doctor, evaluating the results, case and past history, decided on thrombolysistreatment out of hospital while accepting the higher risk of bleeding. Observing the development, the suspected diagnosis has been confirmed. The patient survived the incident and one year later she has completely recovered.
Zusammenfassung
Die Kasuistik schildert die erfolgreiche präklinische Lyse mit Reteplase (Rapilysin®) bei einer 66-jährigen Patientin mit einer fulminanten Lungenembolie unter cardiopulmonaler Reanimation. Die cardial gesunde Patientin wurde mit Herz-Kreislauf- und Atemstillstand aufgefunden. Nur unter cardiopulmonaler Reanimation und direkt nach Suprarenin war ein cardialer Auswurf mit tastbaren Pulsen nachweisbar, sonst lag im EKG bei regelmäßiger Sinustachykardie eine elektromechanische Entkopplung vor. Da unter den Maßnahmen keine Stabilisierung des Kreislaufes erreicht werden konnte, entschloss sich der Notarzt aufgrund der vorliegenden Befunde, der aktuellen Anamnese und der Vorgeschichte zur präklinischen Lyse auch ohne weitere diagnostische Hilfsmittel unter Inkaufnahme des Blutungsrisikos. Der Verlauf bestätigte die Verdachtsdiagnose. Die Patientin überlebte das Ereignis ohne Residuen und ist ein Jahr danach wieder voll leistungsfähig.
