Zusammenfassung
Das Myxödemkoma stellt eine seltene, lebensbedrohliche Komplikation der Hypothyreose dar und sollte in differenzialdiagnostische Überlegungen bei Komapatienten eingehen. Im vorliegenden Fall wurde ein 66-jähriger Patient mit CO2Narkose,Hypothermie,Bradykardie,Hyporeflexie,Tetraparese, Aszites,Pleuraergüssen und Herzinsuffizienz vorgestellt.Nach Normalbefunden im Liquor, kranialem CT,MRT und MR-Angiographie wurde eine Behandlung mit Verdacht auf ein Myxödemkoma durchgeführt. Diese bestand in einer Hochdosistherapie mit L-Thyroxin und Hydrokortison.Letzteres sollte eine sekundäre Nebennierenrindeninsuffizienz verhindern. Die Diagnose wurde bestätigt durch: rasche klinische Erholung, erniedrigtes T3 (0,93 ng/l) und T4 (7,2 ng/l), erhöhtes TSH (20,19 mU/l) und die nachfolgende Anamnese einer Radiojodtherapie mit unregelmäßiger L-Thyroxin-Einnahme.Zusammenfassend sollte bei Verdacht auf ein Myxödemkoma nach Bestätigung durch Labortests unverzüglich mit der Therapie begonnen werden, da der Verlauf der Erkrankung entscheidend vom Zeitpunkt der Behandlung abhängt.
Summary
Myxedema coma is a rare and life-threatening complication of untreated hypothyroidism. Therefore, it must be part of the differential diagnosis in comatose patients. We report one patient who presented with CO2 narcosis,hypothermia, bradycardia,hyporeflexia, tetraparesis, ascitis, pleural effusions, and heart insufficiency.Examination of the CSF, cranial CT, MRI, and MR angiography were normal. In suspicion of myxedema coma,the patient was treated with highdose L-thyroxine and hydrocortisone for preventing secondary adrenal insufficiency. A fast clinical recovery, decreased T4 (7,2 ng/l) and T3 (0,93 ng/l), and increased TSH (20,19 mU/l) together with the following anamnesis of radio iodine therapy and insufficient thyroxine intake confirmed the diagnosis. In conclusion, treatment of the myxedema coma must be started as soon as the laboratory results are confirmatory, since its course depends on the time of initiation of treatment.
