Zusammenfassung

Lebensbedrohliche Kindernotfälle stellen Teams in Klinik und Praxis aufgrund ihrer Komplexität vor große Herausforderungen. Während für den Rettungsdienst in Deutschland die Ausrüstung mit einer DIN-Norm geregelt ist, existieren bis heute keine einheitlichen Empfehlungen für die Vorhaltung von Medikamenten und Ausrüstungsgegenständen für diese Notfälle im innerklinischen Bereich oder in der Kinderarztpraxis. Dabei ist die Akutversorgung pädiatrischer Notfälle ureigene Aufgabe der Kinder- und Jugendmedizin. Im vorliegenden Beitrag werden ausrüstungsrelevante Gesichtspunkte aus den aktuellen Leitlinien zur Erstversorgung lebensbedrohlicher Kindernotfälle des ERC („European Resuscitation Council“) extrahiert. Um die Systematik dieser Leitlinien beizubehalten, wird die hier beschriebene Ausstattung nach dem ABC-Schema eingeteilt. Somit finden wichtige Aspekte zum Atemwegsmanagement (A: „airway“) sowie zur Beherrschung von respiratorischen Problemen (B: „breathing“) ebenso Erwähnung wie Medikamente und Ausrüstungsgegenstände zur Therapie einer schweren Kreislaufinsuffizienz (C: „circulation“). Darüber hinaus werden andere notfallmedizinisch relevante Krankheitsbilder wie der prolongierte epileptische Anfall und die schwere allergische Reaktion bezüglich der dafür notwendigen Ausrüstung berücksichtigt.

Abstract

Life-threatening pediatric emergencies are complex and challenging for medical teams in both pediatric hospital and non-hospital environments. In Germany there is an existing standard for the equipment used by pre-hospital emergency medical services. There are however no standardized recommendations for the equipment required for in-hospital pediatric emergencies or emergencies in non-hospital-based pediatric practices. This article will highlight equipment-related recommendations for the management of life-threatening events in children in these environments. The ABC classification of the European Resuscitation Council (ERC) Resuscitation Guidelines will be used to categorize the recommended equipment. Important aspects for the management of airway (“A”), breathing (“B”) difficulties, as well as practical issues for the treatment of severe circulation (C) compromise in children will be addressed. Unique equipment requirements for managing other pediatric emergencies such as severe allergic reaction and status epilepticus will also be described.
