Zusammenfassung
Die Pharmakotherapie bei älteren Menschen ist für den Arzt oft eine schwierige und nicht selten auch unterschätzte Aufgabe. Viele Medikamente sind für diese Patientengruppe hinsichtlich ihrer Risiko-Nutzen-Bilanz nicht ausreichend untersucht, spezielle unerwünschte Arzneimittelwirkungen wie Sturzereignisse und das delirante Syndrom treten häufiger und klinisch bedeutsamer auf. Schließlich besteht die Problematik der Polypharmazie bei Multimorbidität. Die richtige Balance zwischen unangemessener Übertherapie auf der einen und eine Untertherapie bezüglich eindeutig nützlicher Therapien auf der anderen Seite zu finden, ist nur möglich auf dem Boden einer umfassenden individuellen Risiko-Nutzen-Analyse. Diese muss neben den Präferenzen des Patienten auch seine Vulnerabilität, die verbliebenen Ressourcen und seine Funktionalität mit einbeziehen. Einfache Therapieregeln, aber auch eine verbesserte Kennzeichnung der Medikamente und pharmakotherapeutischen Strategien können hier hilfreich sein.
Abstract
Pharmacotherapy in the elderly is a challenge as the real risk-benefit ratio of many drugs and treatment strategies often remains unknown in these patients. Furthermore there are some special adverse drug reactions such as falls and delirium not only occurring more often but also causing an increased proportion of adverse outcomes. Finally there is the phenomenon of multimorbidity and polypharmacy responsible for treatment errors and inadequate drug treatment. A comprehensive risk-benefit analysis is needed to avoid both under- and over-treatment. This analysis should cover patients preferences, vulnerability, resources and functionality. Some simple treatment rules may improve drug treatment in the elderly, but an improved characterization of drugs and treatment strategies with regard to the elderly is essential.
