Zusammenfassung

Die Anatomie des Schädels und seiner Weichteile ist komplex, der Schädelknochen steht eng benachbart zu vitalen neuronalen Strukturen und sensorischen Organen. Das Gesicht drückt Emotionen ebenso aus wie wichtige Persönlichkeitsmerkmale; dementsprechend führen Verletzungen oft sowohl zu funktionellen als auch zu ästhetischen und persönlichen Problemen. Patienten mit Mittelgesichtstraumata weisen oft zusätzliche Verletzungen anderer Körperregionen auf. Aus diesen Gründen dürfen diese Patienten heute ein klar strukturiertes, interdisziplinäres Konzept der speziellen Diagnostik und Therapie erwarten.

Die vorliegende Arbeit gibt einen Überblick über die Grundsätze der interdisziplinären Behandlung von Patienten mit Mittelgesichtsverletzungen. Die sich überlappenden Therapieangebote der beteiligten Disziplinen werden aus dem Blickfeld der HNO-Chirurgie dargelegt. Grundlegende Kenntnisse der Traumatologie werden vorausgesetzt. Unter Hinweis auf die aktuelle Literatur werden die wichtigsten interdisziplinären Behandlungs-Strategien und spezielle Aspekte der HNO-Traumatologie besprochen. Der HNO-Chirurg ist aufgerufen, in einem multidisziplinären Trauma-Team eine aktive und kooperative Rolle zu spielen.

Abstract

The anatomy of the craniofacial skeleton is complex, the bones being closely associated with important neurological structures and sensory organs. The face itself is intimately associated with hereditary features and emotional feelings, and, as a consequence, midfacial trauma may lead to important aesthetic, functional and personal problems. Patients suffering from severe midfacial injuries reveal significant concomitant lesions of other body regions in a high percentage of cases. Thus, midfacial trauma patients require an elaborate interdisciplinary regimen of diagnostics and therapy.

This paper provides on overview of the interdisciplinary management of midfacial trauma patients. The overlapping multispecialty fields are analyzed with emphasis on the viewpoint of the practicing ENT-surgeon. Assuming that basic principles of trauma care are known, interdisciplinary strategies and specific responsibilities for problem solving are provided. The ENT-surgeon is guided and called upon to play an active and cooperative part in the midfacial traumatology team.

  1. Auswertung auf Basis der fachabteilungsbezogenen Diagnosestatistik L4 der Anlage 1 zur BPflV (Leistungs- und Kalkulationsaufstellung—LKA).

  2. Die tatsächliche Zahl der behandelten Patienten für das Jahr 2002 liegt unterhalb des hier ausgewiesenen Wertes. Die Zählweise in der LKA nach Aufenthalt in einer Fachabteilung führt bei internen Verlegungen zu einer Erhöhung der Zahl an Aufenthalten.
