Integration of the radiotherapy irradiation planning in the digital workflow

Zusammenfassung

Hintergrund und Ziel

Die Klinik für Strahlenheilkunde des Universitätsklinikums verfolgt das Ziel, alle relevanten Arbeitsabläufe als volldigitale Prozesse abzubilden. Nach der erfolgreichen Implementierung unseres neuen Betriebsablaufsystems (BAS), wurden nun alle Verfahren in das Abteilungssystem MOSAIQ als rein digitale Prozesse integriert. Nach der Integration von Behandlungsplanung und Ordination, sollte die organisatorische Abbildung der Bestrahlungsplanung im System erfolgen. Dieses Vorhaben stellt für ein Prozessmanagement eine der größten Herausforderungen dar, da zum einen für diesen komplexen Prozess klar definierte Übergänge (Schnittstellen) zu den beteiligten Berufsgruppen festgelegt sein müssen und zum anderen die Reproduzierbarkeit und Validität aller Teilprozesse gewährleistet sein müssen. Für jeden Abschnitt des Verfahrens sind laut Strahlenschutzverordnung eindeutige und nachvollziehbare Verantwortlichkeiten relevant zu benennen und in der Dokumentation sicher zu stellen. Die speziellen Anforderungen an die digitale Datenhaltung, besonders im Hinblick auf die Langzeitarchivierung und -verfügbarkeit, sind bereits im Rahmen unseres Großprojektes BAS erfüllt.

Methode

Die vorhandenen Abläufe und gesetzlichen Anforderungen der Bestrahlungsplanung wurden durch eine interdisziplinäre Arbeitsgruppe (BAS-AG) im Detail analysiert und nach der Überprüfung und Bewertung der Abbildungsmöglichkeiten in unserem Betriebsablaufsystem in ein Flussdiagramm umgesetzt. Die erarbeiteten Vorgaben wurden durch die klinische und administrative Informatik (KAI) der Klinik für Strahlenheilkunde in einer Testumgebung umgesetzt und nach genauer Prüfung der Lauffähigkeit in den Routinebetrieb gebracht.

Ergebnisse und Schlussfolgerung

Der Abteilung ist es unter Erfüllung der oben genannten Kriterien gelungen, auch den Workflow der Bestrahlungsplanung in das Abteilungssystem als kontinuierlichen Prozess einzubinden. Nach der Fertigstellung des Konzepts und Implementierung in unserer Testumgebung erhielten wir nach der Überprüfung durch das für uns zuständige Regierungspräsidium die Zustimmung für den Einsatz des Verfahrens als volldigitalen Gesamtprozess.

Abstract

Background and purpose

At the Clinic of Radiotherapy at the University Hospital Freiburg, all relevant workflow is paperless. After implementing the Operating Schedule System (OSS) as a framework, all processes are being implemented into the departmental system MOSAIQ. Designing a digital workflow for radiotherapy irradiation planning is a large challenge, it requires interdisciplinary expertise and therefore the interfaces between the professions also have to be interdisciplinary. For every single step of radiotherapy irradiation planning, distinct responsibilities have to be defined and documented. All aspects of digital storage, backup and long-term availability of data were considered and have already been realized during the OSS project.

Method

After an analysis of the complete workflow and the statutory requirements, a detailed project plan was designed. In an interdisciplinary workgroup, problems were discussed and a detailed flowchart was developed. The new functionalities were implemented in a testing environment by the Clinical and Administrative IT Department (CAI). After extensive tests they were integrated into the new modular department system.

Results and conclusion

The Clinic of Radiotherapy succeeded in realizing a completely digital workflow for radiotherapy irradiation planning. During the testing phase, our digital workflow was examined and afterwards was approved by the responsible authority.
