Zusammenfassung

Einleitung

Der Ablauf von Prozessen in der Notaufnahme ist häufig entscheidend für den Behandlungserfolg und bedarf des Qualitätsmanagements. Ein systematisches Erfassen von Qualitätsindikatoren wird in Deutschland seit vielen Jahren gefordert. Eine Grundlage zur Erfassung von Qualitätsindikatoren ist die systematische und möglichst elektronische Dokumentation. Die Sekundärdatennutzung der in einem elektronischen Notaufnahmedokumentationssystem erhobenen Daten des Datensatzes „Notaufnahme“ der DIVI in einem nationalen Notaufnahmeregister ist, neben der Etablierung eines Qualitätsmanagements für Notaufnahmen, Ziel des Projekts „Verbesserung der Versorgungsforschung in der Akutmedizin in Deutschland durch den Aufbau eines Nationalen Notaufnahmeregisters“ (AKTIN). Ziel der Arbeit ist die Identifikation international angewendeter Qualitätsindikatoren in Notaufnahmen.

Methode

Zur Untersuchung der internationalen Situation hinsichtlich eines systematischen Qualitätsmanagements in Notaufnahmen auf der Basis von Qualitätsindikatoren wurde eine systematische Literaturrecherche durchgeführt. Hierzu wurde in PubMed und der Cochrane Library recherchiert sowie eine ergänzende Internetrecherche durchgeführt.

Ergebnis

Aus 20 Publikationen wurden 170 publizierte Qualitätsindikatoren identifiziert, von denen 25 Indikatoren eine mindestens 2‑fache Nennung aufwiesen. Insgesamt 10 Qualitätsindikatoren lassen sich mit dem Datensatz Notaufnahme erfassen. Eine Erweiterung des Datensatzes ermöglicht die Erfassung von 7 weiteren Qualitätsindikatoren. Die Einbindung von Behandlungsdaten über die Notaufnahme hinaus kann zusätzlich die Erfassung von 8 weiteren Qualitätsindikatoren ermöglichen.

Diskussion

Diese Arbeit konnte darstellen, dass das Potenzial des nationalen Notaufnahmeregisters zur Etablierung von Qualitätsindikatoren dem der international betrachteten Systeme entspricht und eine vergleichbare Abbildung von Qualitätsdimensionen ermöglichen kann.

Abstract

Background

Emergency department processes are often key for successful treatment. Therefore, collection of quality indicators is demanded. A basis for the collection is systematic, electronic documentation. The development of paper-based documentation into an electronic and interoperable national emergency registry is—besides the establishment of quality management for emergency departments—a target of the AKTIN project. The objective of this research is identification of internationally applied quality indicators.

Methods

For the investigation of the current status of quality management in emergency departments based on quality indicators, a systematic literature search of the database PubMed, the Cochrane Library and the internet was performed.

Results

Of the 170 internationally applied quality indicators, 25 with at least two references are identified. A total of 10 quality indicators are ascertainable by the data set. An enlargement of the data set will enable the collection of seven further quality indicators. The implementation of data of care behind the emergency processes will provide eight additional quality indicators.

Conclusion

This work was able to show that the potential of a national emergency registry for the establishment of quality indicators corresponds with the international systems taken into consideration and could provide a comparable collection of quality indicators.
