Zusammenfassung

Frailty (Gebrechlichkeit) ist das zentrale altersmedizinische Syndrom. Auf der Basis von Alter, Alterungsprozessen, chronischen Erkrankungen, Multimorbidität und Polymedikation kommt es zu funktionellen Einbußen mit einer erhöhten Vulnerabilität und einer erhöhten Wahrscheinlichkeit von Morbidität, Institutionalisierung, Pflegebedürftigkeit und Tod. Das Frailty-Syndrom umfasst physische, psychische und soziale Aspekte. In den Screeningverfahren werden zwischen 5 bis 70 Parameter berücksichtigt. Die zentralen physischen Aspekte der Frailty beinhalten Gewichtsabnahme, eingeschränkte Belastbarkeit, motorische Kraft, Geschwindigkeit und Ausdauer. Prophylaxe und Therapie des Frailty-Syndroms beinhalten die Behandlung der aktiven Komorbiditäten, Optimierung der Medikation und des Ernährungsstatus in Kombination mit Physiotherapie und ADL-Training sowie Berücksichtigung von Umgebungsfaktoren. Auf diese Weise können Aktivität, Mobilität, soziale Teilhabe und Lebensqualität positiv moduliert werden.
