Toxische und allergische Kontaktdermatitis

Zusammenfassung

Kontaktdermatitis ist eine nichtinfektiöse entzündliche Dermatose, bei der pathologische Veränderungen in der Epidermis und im oberen Korium das klinische Bild prägen. Ätiopathogenetisch handelt es sich um eine exogen ausgelöste, oft durch eine individuelle Disposition begünstigte, irritative oder allergische Reaktion auf den Kontakt mit einer Noxe. Unabhängig von der Ätiopathogenese finden sich ähnliche klinische, histologische und molekulare Veränderungen. Kontaktdermatitis – einschließlich der photoinduzierten Sonderformen – gehört mit atopischem Ekzem, seborrhoischem Ekzem und nummulärer Dermatitis zum Formenkreis der Dermatitis- oder Ekzemerkrankungen. Diese sind häufig: etwa 15–20 % der dermatologischen Patienten sind betroffen. Die Nomenklatur der Dermatitis- oder Ekzemerkrankungen ist uneinheitlich, weil bislang weder ätiologisch noch pathogenetisch eine gesicherte Klassifikation möglich ist. Die Diagnosen Dermatitis oder Ekzem werden vielfach synonym gebraucht, wobei sich im angloamerikanischen Raum der Begriff Dermatitis weitgehend durchgesetzt hat. Traditionell wurden in der deutschsprachigen Fassung dieses Buchs Erkrankungen mit akutem Verlauf und rascher Rückbildungsfähigkeit als Dermatitis und solche mit chronischem Verlauf und geringer Spontanregressionstendenz als Ekzem bezeichnet, wobei Übergänge und Zwischenformen vorkommen.
