Zusammenfassung
Gerade in der Geburtshilfe kommt dem Fehler- und Risikomanagement eine wachsende Bedeutung zu. Die Fachliteratur sowie alle bisher gemachten Erfahrungen weisen darauf hin, dass der beste Weg darin besteht, sich an den Sicherheitsstrategien der „high reliability organisations“ (HRO: Luftfahrt, Raumfahrt, Petrochemie, Nuklearkraft), die zu den „ultra-safe technologies“ zählen, zu orientieren. Auslöser für die Beschäftigung mit vermeidbaren Fehlern und deren Folgen war das Buch „To Err is Human“ im Jahr 1999. Die wesentlichste Schlüsselbotschaft war, dass sowohl in den HRO als auch in der Medizin 80 % aller Komplikationen auf Team- bzw. Kommunikationsdefizite zurückzuführen sind. Den weitaus größten Effekt in der Vermeidung von Fehlern haben demnach all jene Aktivitäten, die die Kommunikation und Teamarbeit verbessern. Den kritischen Erfolgsfaktor schlechthin bildet dabei die herrschende Fehlerkultur. Ein neuer Wissenszweig „patient safety science“ befindet sich in der Entwicklung.
