Weder AZA noch sein aktiver Metabolit 6-Mercaptopurinribonukleotid (6-MP,s. unten) sind direkt immunomodulatorisch wirksam: 6-MP wird enzymatisch durch die Xanthinoxidase in 6-Thioharnsäure (Cave: Allopurinoll), die Thiopurinmethyltransferase (TPMT) in 6-Methyl-Mercaptopurin (Cave: TPMT-Defizienz) oder alternativ durch die Hypoxanthin-Phosphoribosyltransferase in 6-Thioninosin-5-Monophosphat umgewandelt. Methylierte Metaboliten letzteren Produktes sind wahrscheinlich nach Einbau in DNA und RNA als Antimetaboliten wirksam, können zudem die De-novo-Purinsynthese hemmen und durch den Purinmangel indirekt ebenfalls die DNA- und RNA-Synthese beinträchtigen. Mangels eines »salvage pathway« wird die Lymphozytemproliferation samt der humoralen und zellulären Immunant gehemmt.
