Fallstricke bei der intraossären Punktion

Zusammenfassung

Der intraossäre Zugang hat in den letzten Jahren sowohl in der Anwendung als auch in der Entwicklung neuer Techniken eine Renaissance erlebt. Ursprünglich als Routineverfahren in der Pädiatrie verwendet, wird der intraossäre Zugang mittlerweile erfolgreich in Notfallsituationen, bei denen ein konventioneller venöser Zugang nicht oder nur zeitlich verzögert gelingt, bei Kindern wie auch Erwachsenen angewandt. In einigen Arbeiten wird die unkomplizierte Handhabung und hohe Erfolgsrate propagiert.
Die vorliegende Kasuistik berichtet über den Fall einer 67-jährigen Patientin, bei der aufgrund einer arteriellen Blutung und schwieriger venöser Verhältnisse ein intraossärer Zugang mit zwei unterschiedlichen Systemen erfolglos versucht wurde. Der Fall zeigt, dass trotz der einfachen Handhabung der intraossären Punktion vor allem beim Erwachsenen Detailwissen sowohl bezüglich der Anatomie als auch der Technik der verschiedenen Systeme notwendig ist. In der Diskussion wird auf mögliche Fallstricke bei der intraossären Punktion hingewiesen.
