Ein Pilotprojekt zur erweiterten Notfalltherapie durch Rettungsassistenten

Zusammenfassung

Empfehlungen der Bundesärztekammer sehen den Einsatz ausgewählter Notfallmedikamente durch Rettungsassistenten auch vor Eintreffen eines Arztes vor. Daten zur Sicherheit und Praktikabilität im deutschen Rettungsdienst liegen bislang nicht vor. In einem Pilotprojekt wurden Rettungsassistenten weiterqualifiziert, um algorithmisierte Versorgungsaufträge einschließlich medikamentöser Therapie selbständig durchführen zu können. Methodik. Nach einem strukturierten Auswahlverfahren wurden 15 Rettungsassistenten mit Berufserfahrung ≥3 Jahren in der Anwendung spezifischer Behandlungsalgorithmen (Extremitätentrauma, Bewusstseinsstörung, akutes Koronarsyndrom) geschult. Die Schulung bestand aus einer eintägigen theoretischen Unterrichtung, Training im Anästhesievollsimulator sowie einem 3-tägigem Anästhesiepraktikum und Lernzielkontrolle. Über ein „Call-back-System“ war einer der beiden projektbetreuenden Notärzte ständig erreichbar. Ergebnisse. Im Untersuchungszeitraum 01.01.–31.12.2005 wurden insgesamt 38 Patienten (Trauma 25, akutes Koronarsyndrom 10, Hypoglykämie 3) gemäß Algorithmen versorgt. Von 25 Patienten mit Trauma konnte bei 9 allein durch achsengerechte Extension eine ausreichende Schmerzlinderung erzielt werden. 16 Patienten erhielten Morphin i.v. (Mittel 8,6±4,4 mg), davon erfolgte bei 7 Patienten eine Weiterbehandlung durch den Notarzt, 9 Patienten wurden nach Analgesie ohne notärztliche Behandlung in die Klinik transportiert. Es wurden keine relevanten Komplikationen dokumentiert. Schlussfolgerungen. Die durchgeführte Qualifizierungsmaßnahme erscheint geeignet, ausgewählte Rettungsassistenten mit der Anwendung erweiterter medizinischer Maßnahmen vor Behandlungsübernahme durch den Arzt zu beauftragen.
