Intramuskuläre Adrenalingabe durch Zahnärzte bei Anaphylaxie

Hintergrund

Anaphylaktische Reaktionen zählen zu den typischen systemischen Komplikationen bei der Anwendung von Medikamenten und zahnärztlichen Materialien. Zahnärzte sollten sie daher frühzeitig erkennen und behandeln können. Ohne Zweifel spielt die frühzeitige Gabe von Adrenalin eine entscheidende Rolle bei der Behandlung der Anaphylaxie. Für den notfallmedizinisch Ungeübten stellt sich jedoch häufig die Frage nach Applikationsart und Dosis, über die unterschiedliche Empfehlungen kursieren. Ziel unserer Untersuchung war es festzustellen, ob die intramuskuläre (i.m.) Adrenalinapplikation für Zahnärzte ein sicheres Verfahren darstellt.

Material und Methoden

Mittels Hochfrequenzultraschall wurde die Dicke der Gewebsschichten bis zur Muskulatur (M. vastus lateralis) bei 100 Probanden gemessen, um die notwendige Punktionstiefe für eine sichere i.m.-Injektion festzustellen. Die Durchführbarkeit einer konventionellen i.m.-Injektion in den M. vastus lateralis evaluierten wir mit 106 Zahnärzten an einem Simulator für diese Injektionsform.

Ergebnisse

Die durchschnittliche Dicke der Kutis war bei beiden Geschlechtern annähernd gleich (Frauen: 1,50±0,16 mm, Männer: 1,54±0,14 mm), die der Subkutis war mit 9,47±0,73 mm bei den Frauen deutlich dicker als bei den Männern (3,93±0,42 mm). Bei den Frauen beginnt die Muskelschicht damit 10,95±0,68 mm, bei den Männern 5,48±0,39 mm unter der Hautoberfläche. Von 106 Zahnärzten waren nach einmaliger kurzer Einweisung 95% in der Lage, eine korrekte i.m.-Injektion mit Einmalspritze und -kanüle in den M. vastus lateralis durchzuführen.

Schlussfolgerung

Die konventionelle Punktion des M. vastus lateralis ist eine leicht zu erlernende Technik mit hoher Erfolgsquote, die bei einer Punktionstiefe von mindestens 20 mm eine sichere i.m.-Injektion bei den meisten Erwachsenen erlaubt. Diese Technik sollte daher als Applikationsform für Adrenalin bei Anaphylaxie für notfallmedizinisch Unerfahrene bevorzugt werden. Die Länge der Injektionsnadel eines handelsüblichen Autoinjektors (Anapen®, Fastjekt®) erscheint nicht ausreichend lang für einen Großteil der Erwachsenen.
