Interventionen zur Förderung der körperlichen Aktivität in Pflegeheimen

Systematische Übersicht der Wirksamkeit universeller Prävention

Zusammenfassung

Hintergrund

Körperliche Aktivität könnte die gesundheitsbezogene Lebensqualität stationär Pflegebedürftiger erhalten, da eine Verringerung hier insbesondere auf Einbußen der körperlichen Funktionsfähigkeit beruht. Ziel ist die Evaluation der Wirksamkeit von universell präventiven Angeboten zur Förderung von körperlicher Aktivität auf die Durchführbarkeit der Aktivitäten des täglichen Lebens bei Pflegebedürftigen in der stationären Pflege.

Material und Methode

Relevante Literatur wurde über eine systematische Recherche in den Datenbanken MEDLINE, The Cochrane Library, EMBASE, CINAHL, PsycINFO und PEDro identifiziert. Datenextraktion und Prüfung des Verzerrungsrisikos wurden durch 2 Autoren unabhängig voneinander vorgenommen. Die Ergebnissynthesen beruhen auf Metaanalysen mit „Random-effects“-Modellen.

Ergebnisse

Unter Einbezug von 14 Primärstudien weisen stationär Pflegebedürftige nach Teilnahme an körperlichen Aktivitäten im Vergleich zu verschiedenen Kontrollbedingungen eine statistisch signifikant bessere körperliche Funktionsfähigkeit auf (Standardisierte Mittelwertdifferenz [SMD] = 0,48, 95 %-Konfidenzintervall [95 %-KI] 0,26–0,71, p < 0,0001). Subgruppenanalysen lassen vermuten, dass insbesondere stationär Pflegebedürftige mit vermehrten körperlichen und kognitiven Beeinträchtigungen von körperlicher Aktivität profitieren können. Ergebnisse nach einem trainingsfreien Zeitraum begründen die Notwendigkeit einer dauerhaften Implementierung. Aufgrund des insgesamt hohen Verzerrungsrisikos der Studien ist die Aussagekraft der Ergebnisse allerdings limitiert.

Schlussfolgerung

Die Ergebnisse deuten auf eine Effektivität von körperlicher Aktivität in der stationären Pflege hin. Angesichts einer niedrigen Evidenzgüte sind qualitativ hochwertige Studien für eine Überprüfung der Robustheit der statistischen Ergebnisse notwendig.
