Zusammenfassung

Herz-Kreislauf-Krankheiten stehen weltweit an erster Stelle der Todesursachen. Die Hochrechnung repräsentativer Werte für Inzidenz und Überleben nach Herzstillstand bei sämtlichen Rhythmen mit notfallmäßiger medizinischer Versorgung („out-of-hospital cardiac arrest“, OHCA) ergibt etwa 350.000 Personen in Europa, bei denen ein OHCA auftritt [10].
Die oben erwähnte bedeutende Zahl und die Tatsache, dass der Einfluss einzelner therapeutischer Maßnahmen auf die Verbesserung des individuellen Überlebens nicht bekannt ist, erfordert Standardmaßnahmen, die zum bestmöglichen Ergebnis für die größtmögliche Anzahl Patienten führen. Neben den lebensrettenden Basismaßnahmen haben sich nur wenige erweiterte lebensrettende Maßnahmen definitiv als wertvoll für die Wiedererlangung des Spontankreislaufs („return of spontaneous circulation“, ROSC) und für ein verbessertes neurologisches Ergebnis herausgestellt: Defibrillation, einige pharmakologische Interventionen und therapeutische Hypothermie.
Das ILCOR (International Liaison Committee on Resuscitation) ist dabei, erneut eine evidenzbasierte Übersicht über wissenschaftliche Daten zur Wiederbelebung zu koordinieren, die Konsensus-Konferenz wird im Februar 2010 stattfinden. Deren Ergebnisse werden dem europäischen Wiederbelebungsrat (European Resuscitation Council, ERC) beim Schreiben der neuen Leitlinien zur Wiederbelebung als Basis dienen. Wie ist der heutige Wissensstand? Was ist an Neuem zu erwarten?
