Zusammenfassung

Die Hämorrhagie ist die zweithäufigste Todesursache polytraumatisierter Patienten. Die Koagulopathie des Traumas ist durch nicht-chirurgische, diffuse Blutungen aus Schleimhaut, Serosa und Wundflächen gekennzeichnet, die im Kontext von schwerer Verletzung, Hypothermie, Azidose, Verdünnung/Verbrauch und dissiminierter intravaskulärer Koagulopathie (DIC) auftreten. Azidose und Hypothermie beeinträchtigen sowohl Thrombozyten und Gerinnungsfaktoren, wie auch die Thrombinbildung. Die einzelnen Faktoren sind dabei allerdings unterschiedlich empfindlich. Da die Prophylaxe dieser Störungen einfacher als die Therapie ist, sollte bereits am Unfallort damit begonnen werden. Aktives und passives Erwärmen auf Normothermie, zumindest jedoch >35°C, eine Begrenzung der Gabe von kristalloidem und kolloidalem Volumenersatz auf einen mittleren arteriellen Druck (MAP) von ≥65 mmHg („hypotensive resuscitation“), die Verhinderung bzw. der Ausgleich einer Azidose auf einen pH-Wert >7,2 bzw. einen „base excess“ (BE) >-12,5 sowie die frühzeitige und ausreichende Gabe von gerinnungsaktiven Medikamenten, in erster Linie von gefrorenem Frischplasma (FFP), Fibrinogen und Thrombozyten, können eine deutliche Verbesserung der Überlebenschancen dieser Patienten ermöglichen.
