Zusammenfassung

Der Begriff Human Factors (HF) wird vielfach im Zusammenhang mit Patientensicherheit und Fehlern in der Medizin – oft auch missverständlich – gebraucht. Tatsächlich umfasst er eine große Bandbreite an Bedeutungen, die von Mensch-Maschinen-Schnittstellen über Leistungsfähigkeit und Limitierungen von Menschen bis hin zur Gestaltung von Arbeitsprozessen reicht. Jedoch gelten HF als wesentliche Fehlerquellen in komplexen Systemen. Dieser Beitrag zeigt die Spannweite des Begriffs „Human Factors“ aus der (notfall-)medizinischen Perspektive. Anhand des „Schweizer-Käse-Modells“ von Reason erklären wir die unterschiedlichen Fehlerarten, was ihre Entstehung begünstigt und auf welcher Ebene des Modells Maßnahmen zur Fehlervermeidung im Sinne einer Qualitätskultur getroffen werden können.
