Die Bedeutung pflegerischer und sozialer Risikofaktoren

Zusammenfassung

Hintergrund

Stürze ziehen bei älteren Menschen häufig Verletzungen und Pflegebedürftigkeit nach sich und stellen eine Herausforderung für die Sicherung der Qualität der pflegerischen Versorgung dar. Das Sturzgeschehen bei ambulant Pflegebedürftigen in Deutschland wird beschrieben, und Risikofaktoren werden identifiziert.

Methode

Die Teilnehmer der Studie „Pflegerelevante Gesundheitsprobleme in der ambulanten Pflege und Versorgung“ (n = 880) wurden auf Basis der zugelassenen ambulanten Pflegedienste rekrutiert. Die Datenerhebung erfolgte nach einem standardisierten Studienprotokoll. Anteilswerte wurden berechnet und adjustierte „odds ratios“ sowie vorhergesagte Häufigkeiten mithilfe binär-logistischer Regressionen geschätzt.

Ergebnisse

Stürze sind in der ambulanten Pflege ein häufiges Ereignis: Fast jeder zehnte Pflegebedürftige stürzt innerhalb von 2 Wochen. Stürze sind v. a. mit sozialen und pflegerischen Faktoren assoziiert. Neben einem geringen Bildungsstand zählt hierzu die Lebens- und Versorgungssituation: Alleinlebende, Personen mit seltenen Kontakten zum Pflegedienst sowie Klienten, die aus pflegerischer Sicht mehr Hilfe benötigen, weisen deutlich erhöhte Chancen zu stürzen auf. Multimorbidität stellt zudem einen medizinischen Risikofaktor dar. Neurologische Erkrankungen und Polypharmazie erwiesen sich dagegen erst im Zusammenwirken mit der Lebens- und Versorgungssituation als risikosteigernd.

Schlussfolgerungen

Pflegefachkräfte und Ärzte sollten die soziale und gesundheitliche Situation Pflegebedürftiger im Blick behalten. Treffen medizinische Problemlagen Alleinlebende oder fallen sie mit einem geringen Versorgungsumfang zusammen, steigt das Risiko für Stürze deutlich. Ändert sich die Lebenssituation, sollten präventive Interventionen und sozialrechtliche Folgebegutachtungen zeitnah erwogen werden.
