Zusammenfassung

Eine 39-jährige Schwangere freut sich auf ihr zweites Kind, nachdem ihr erstes vor knapp 12 Monaten in der 32. Woche mit Kaiserschnitt zur Welt gekommen ist. Auf ihren ausdrücklichen Wunsch soll die zweite Geburt auf natürlichem Weg erfolgen. Sie ist 10 Tage über dem errechneten Termin. Kurz vor der geplanten Geburtseinleitung bekommt sie krampfartige Bauchschmerzen und sucht sofort die Klinik auf. Aus der Anamnese: Z. n. Sectio wegen Beckenendlage, regelmäßige Wehen, wahrscheinlich vorzeitiger Blasensprung. Der Blick in den Mutterpass ergibt in Risikokatalog A: Schwangere über 35 Jahre, Z. n. Frühgeburt, Z. n. Sectio wegen BEL 32. SSW. Dazu kommt die Eintragung der Frauenärztin »Kind männlich, Schätzgewicht um die 4000 g«. Blutdruck, Puls und Urinstatus sind unauffällig. Die Geburt geht zunächst gut voran, doch dann zeichnet sich eine geburtshilfliche Katastrophe ab. Was ist in dieser kritischen Situation zu tun?
