17. Fetale Magnetresonanztomographie

Zusammenfassung

Die fetale Magnetresonanztomographie (MRT) hat in den letzten Jahren erheblich an klinischer Bedeutung gewonnen. Sie wird in der pränatalen Diagnostik als komplementäres Verfahren zur Standardmethode, dem Ultraschall, angewandt. Die meisten fetalen MRT-Untersuchungen werden zur weiteren Klärung von sonographisch auffälligen Befunden des zentralen Nervensystems (ZNS) durchgeführt. Andere Fragestellungen zu Thorax, Abdomen, Urogenitalsystem oder der Plazenta können ebenso eine Indikation zur Durchführung einer MRT sein und seit kurzer Zeit hat auch die Beurteilung des fetalen Herzen Einzug in die MRT-Diagnostik gefunden. Dieses Kapitel erläutert verschiedene Aspekte der fetalen MRT-Bildgebung, die von technischen und sicherheitsrelevanten bis hin zu den diagnostischen Möglichkeiten reichen. Einigkeit besteht, dass die diagnostische Wertigkeit der ergänzenden fetalen MRT in der fortgeschrittenen Schwangerschaft erhöht ist. Ultraschall und MRT haben unterschiedliche Stärken und Schwächen, so dass Methodenvergleiche teilweise inkohärente Ergebnisse zeigen, je nachdem aus welchem Blickwinkel eine Studie geplant wurde. Beide Methoden funktionieren als Ergänzungsverfahren besonderes gut. Für die Durchführung der fetalen MRTUntersuchungen ist daher besonders wichtig, dass eine interdisziplinäre Kooperation und die gemeinsame Diagnosefindung anhand aller verfügbaren Befunde aus Ultraschall und MRT-Bildern vorgehalten wird.
