Der De Morton Mobility Index zur Evaluation der geriatrischen Frührehabilitation

Hintergrund

Zur Dokumentation des Rehabilitationserfolgs werden in der geriatrischen Frührehabilitation am häufigsten der Timed up and go (TUG-)Test und der Tinetti-Test eingesetzt. Es erscheint jedoch fraglich, ob sie in diesem Setting das gesamte Mobilitätsspektrum mit ausreichender Präzision abbilden können und zur Verlaufsbeurteilung geeignet sind. Vor diesem Hintergrund wurde der De Morton Mobility Index (DEMMI) entwickelt und validiert.

Ziel der Arbeit

Ziel war es, die Wertigkeit des DEMMI gegenüber TUG- und Tinetti-Test für die geriatrische Frührehabilitation zu untersuchen.

Material und Methode

Decken- und Bodeneffekte von DEMMI, TUG-, Tinetti-Test und Barthel-Index (BI) wurden ermittelt. Korrelationen wurden mithilfe des Spearman-Korrelationskoeffizienten berechnet. Einflüsse auf den DEMMI wurden mit multipler linearer Regression geschätzt.

Ergebnisse

Es wurden 144 Patienten (Alter 84,91 Jahre [SD ±5,77 Jahre], 65,97 % weiblich) untersucht. Beim DEMMI wurden im Mittel 33,45 Punkte (SD ±15,37 Punkte) bei Aufnahme und 43,90 Punkte (SD ±15,55 Punkte) bei der Entlassung (jeweils n = 144) erreicht. Bodeneffekte beim TUG-Test wurden bei Aufnahme bei 84,72 % (n = 122) und bei der Entlassung bei 60,42 % der Patienten (n = 87) beobachtet. Keine Boden- oder Deckeneffekte zeigten hingegen DEMMI, Tinetti-Test und BI. Die Korrelation bei Aufnahme und Entlassung betrug zwischen DEMMI und TUG-Test −0,48 und −0,49, zwischen DEMMI und Tinetti-Test 0,75 und 0,82 sowie zwischen DEMMI und BI 0,54 und 0,66 jeweils mit p < 0,01.

Diskussion

Signifikante Korrelationen zu TUG-, Tinetti-Test und BI lassen darauf schließen, dass der DEMMI ein geeignetes Instrument ist, um den Erfolg der geriatrischen Frührehabilitation abzubilden. Da keine Boden- und Deckeneffekte auftreten, scheint der DEMMI dem TUG-Test überlegen.
