Das Gerinnungssystem in der S3-Leitlinie Polytrauma/Schwerverletzten-Behandlung 2016

Update über die neusten Konzepte der suffizienten Blutstillung

Zusammenfassung

Neben dem schweren Schädel-Hirn-Trauma sind starke Blutungen eine der typischen Todesursachen beim Trauma. Hämorrhagien zählen daneben zu den häufigsten vermeidbaren Todesursachen, dies unterstreicht die Wichtigkeit einer frühen und konsequenten Blutstillung. Die aktuelle S3-Leitlinie „Polytrauma/Schwerverletzten-Behandlung“ weist daher auch in verschiedenen Bereichen auf die Wichtigkeit und Möglichkeiten einer suffizienten Blutstillung hin. Neben der reinen Blutung ist beim Trauma auch eine Störung des „Organsystems Gerinnung“ (traumainduzierte Koagulopathie [TIK]) häufig, weshalb die aktuelle Leitlinie diesem Bereich ein eigenes Kapitel widmet, das im Vergleich zur Vorgängerversion aus dem Jahr 2011 deutlich überarbeitet und erweitert wurde. Entscheidend sind die frühe Diagnostik und Therapie der Gerinnungsprobleme, wobei diagnostisch neben der frühzeitigen Bestimmung von Globaltests der Gerinnung und von Schockparametern viskoelastische Testverfahren deutlich an Bedeutung gewinnen. Etabliert werden sollen Massivtransfusions- und Gerinnungsprotokolle, die an die Strukturen der Kliniken angepasst sind. Therapeutisch ist auf die Optimierung der Rahmenbedingungen Normovolämie, Normothermie, Normokalzämie und ausgeglichener Säure-Basen-Haushalt zu achten, wenn dies auch oft schwierig ist. Für die Therapie der TIK stehen neben der sog. „damage control resuscitation“ mit dem verhältnisbasierten und dem faktorkonzentratbasierten Konzept unterschiedliche Regime zur Verfügung. Unabhängig vom bevorzugten Konzept sollte beim blutenden Schwerverletzten an den frühen Einsatz von Tranexamsäure gedacht werden. Die vorliegende Arbeit fasst die wichtigsten Punkte des Kapitels zusammen und geht auf aktuelle Publikationen zum Thema ein.
