Bakterielle Meningitis bei Erwachsenen im Not- und Rettungswesen

Zusammenfassung

Die Kardinalsymptome der bakteriellen Meningitis sind Kopfschmerzen, Fieber, Vigilanzminderung und Nackensteife (Meningismus). Allerdings kann das Vorliegen einer Meningitis definitiv nur durch die Untersuchung des Liquors diagnostiziert bzw. ausgeschlossen werden. Ein wichtiger beeinflussbarer Faktor für den weiteren Krankheitsverlauf ist der schnelle Beginn einer antibiotischen Therapie. Bei ambulant erworbener akuter bakterieller Meningitis des Erwachsenen in Deutschland besteht die empirische Therapie in den Gaben von Ceftriaxon und Ampicillin sowie adjuvant Dexamethason. Hiermit soll unmittelbar im Anschluss an die Lumbalpunktion gestartet werden. Im Fall einer Zeitverzögerung (z. B. aufgrund der Notwendigkeit, vor der Lumbalpunktion ein Schädel-CT durchzuführen), sollte die antibiotische Therapie noch vor der Gewinnung des Liquors begonnen werden.
