Zusammenfassung

Die Therapie der symptomatischen Aortenklappenstenose (AS) durch einen konventionellen Aortenklappenersatz (AKE) oder eine „transcatheter aortic valve implantation“ (TAVI) hat auch für Patienten im fortgeschrittenen Alter eine gute Prognose. Eine leitliniengerechte Indikationsstellung und die Entscheidung zum richtigen Therapiepfad durch ein Heart-Team sind erfolgsentscheidend. Gerade bei älteren und zunehmend multimorbiden Patienten ist die Erfassung der funktionellen Defizite nicht nur für die Mortalität, sondern auch für peri- und postoperative Komplikationen sowie das funktionelle Outcome relevant. Die verschiedenen Verfahren des geriatrischen Assessments stellen eine wichtige Ergänzung zu den standardmäßig erhobenen Risiko-Scores dar. Ziel ist es, fokussiert Interventionen einzusetzen, um Risikofaktoren zu minimieren und die Prognose älterer Patienten zu verbessern. Dieser Beitrag beschreibt die gängige Praxis in der Behandlung der Aortenklappenstenose und fasst aktuelle Aspekte geriatrischer Patienten zusammen.
