Zusammenfassung

In Akutsituationen im Rettungsdienst sind Diagnose und Therapie von Herzrhythmusstörungen an einfachen und sicher erkennbaren Kriterien auszurichten. Neben Anamnese und körperlichem Untersuchungsbefund ist v. a. das 12-Kanal-Oberflächen-EKG entscheidend. Die Einteilung in regelmäßige sowie unregelmäßige Schmal- und Breitkomplextachykardien hilft bei der Zuordnung der Herzrhythmusstörung und ist die Grundlage der medikamentösen Therapie. Die elektrische Therapie (Kardioversion/Defibrillation) und die medikamentöse antiarrhythmische Therapie sind maßgeblich in der Behandlung der häufig vital gefährdeten Patienten. Im Rettungsdienst kann die antiarrhythmische Medikation auf wenige sinnvoll einsetzbare Substanzen reduziert werden. Die vorliegende Arbeit beruht auf den aktuellen Therapieempfehlungen des European Resuscitation Council (2015), der Europäischen Gesellschaft für Kardiologie, den amerikanischen Leitlinien (American College of Cardiology, American Heart Association) und einer Literaturauswahl der Autoren.
