Was braucht man für die Diagnose?

Hintergrund

Die Herzinsuffizienz ist eine der häufigsten internistischen Erkrankungen und damit auch ein sehr häufiger notfallmäßiger Aufnahmegrund im Krankenhaus. Meist handelt es sich um eine aus unterschiedlichsten Ursachen dekompensierte chronische Herzinsuffizienz. In der Akutsituation präsentiert sich meist ein akut lebensbedrohter Patient, weshalb Diagnosestellung und Therapie möglichst rasch erfolgen sollten.

Anamnese und Klinik

Da die Symptomatik und Klinik unspezifisch und differenzialdiagnostisch, z. B. bei einer bestehenden pulmonalen Erkrankung, nicht immer leicht abzugrenzen sind, kommt einer genauen Anamnese eine große Bedeutung zu. Kardinalsymptom ist die Dyspnoe, oft verbunden mit thorakalen Schmerzen. Neben der aktuellen Anamnese sollte unbedingt auch die Vorgeschichte bzw. Krankengeschichte des Patienten erfragt werden. Bei der körperlichen Untersuchung sind die Rasselgeräusche eines Lungenödems und Ödeme, v. a. der Unterschenkel, typisch. Bei der Untersuchung sollte unbedingt auf die Anzeichen eines beginnenden oder bereits manifesten kardiogenen Schocks geachtet werden.

Wesentliche Untersuchungen

Wichtigste Untersuchungen sind 12-Kanal-EKG und transthorakale Echokardiographie. Einerseits können hier Herzrhythmus, Herzfrequenz und Erregungsrückbildung, andererseits linksventrikuläre systolische Pumpfunktion, Herzklappen und Dimensionen beurteilt und die weitere Therapie eingeleitet werden. Weitere Untersuchungen wie u. a. Laborparameter und Röntgen-Thorax komplettieren die Diagnostik.

Schlussfolgerung

Die akute Herzinsuffizienz ist ein sehr häufiges und akut lebensbedrohliches Krankheitsbild. Jeder Notfall- und Intensivmediziner sollte sie möglichst zügig erkennen und den Patienten entsprechend behandelnd können.
