Acetylsalicylsäure-Nonresponder nach ischämischem Insult bei geriatrischen Patienten

Zusammenfassung

Hintergrund

Das kardiovaskuläre und zerebral-ischämische Risiko ist definiert als das Risiko, ein Ereignis eines thromboembolischen Geschehens zu erleiden. Die gängige Sekundärprophylaxe ist nach wie vor die Gabe von Acetylsalicylsäure (ASS). Die übliche Tagesdosis beträgt 100 mg. Eine Wirksamkeitskontrolle in Hinblick auf die Thrombozytenaggregation erfolgt in der Routine nicht.

Material und Methoden

In dieser Studie wurden Patienten nach Aufnahme auf der Stroke Unit untersucht, die unter 100 mg Acetylsalicylsäure täglich per os einen Schlaganfall beziehungsweise Schlaganfallrezidiv erlitten hatten. Die Thrombozytenaggregation wurde mittels Platelets Function Analyser (PFA 100) durchgeführt.

Ergebnisse

Insgesamt wurden 71 Patienten untersucht. Davon erlitten 53 (73 %) ein Erstereignis eines ischämischen Insults und 18 (25,4 %) ein Apoplexrezidiv. 14 Patienten (19,7 %) zeigten keine Verlängerung der Verschlusszeit unter Acetylsalicylsäure 100 mg und wurden als Nonresponder bezeichnet.

Schlussfolgerung

Es bleibt offen, ob die reine Umstellung der medikamentösen Sekundärprophylaxe oder eine effizientere Thrombozytenaggregationshemmung eine Besserung des Schutzes vor dem nächsten Ereignis bedingt.
